SUMMARY OF JOB:
Facilitates the flow of patient care throughout the acute hospitalization episode of care, utilizing the care management process. Maintains high quality and efficient service delivery. Functions as a patient advocate and liaison to family, physicians, other healthcare team members, community resources and third party payers. Provides necessary, appropriate interventions that foster independence and wellness, and prevent inappropriate unnecessary admissions and/or utilization of resources. Age specific criteria listed is to assure individual performance demonstrates employee knowledge and skills necessary to provide psycho-social, emotional and educational needs appropriate to the age of the patients served in his/her assignment. The skill and knowledge needed to provide such care might be gained through education, training or experience.

RECRUITMENT REQUIREMENTS:
Degreed Social Worker
Minimum 2 years hospital medical/surgical or home health experience
Working knowledge of managed care and third party reimbursement
Experience with case management, evaluation/assessment of clients
One hour of annual stroke education
Basic computer skills required
Advocacy and problem solving skills
Knowledgeable of community resources, including availability of services
Excellent time management and written and verbal communication
Possesses strong organizational and negotiation skills
Possesses strong team work skills
On call responsibilities required



Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
